Health Form Nights: Wednesday, Jan. 31 and Feb. 7
Get your scout ready for 2024 camping trips and summer camp by attending one of our upcoming health form evenings. Many forms are currently expiring and need to be updated. If you are unable to attend one of these evenings, you will need to complete Part C at your doctor's office or a walk-in clinic. As of now, we do not have any other health form evenings planned.
Dates: January 31 and February 7
Time Blocks: 6:30pm (for adult and youth leadership)

Bring the following:
1. Health form with Parts A & B completed, Part C with Name and DOB
2. A copy of the front and back of your insurance card.
*WALK-INS WELCOME, but we will accommodate those signed up first. We will begin taking the 7:30pm sign ups and walk-ins after the leadership group has finished.
*Health Forms are required for all scouts and scouters that participate in any scouting activity. Health forms are valid for 1 year. Please be sure to update your form before it expires.
