The troop is sponsoring Health Form Nights again this year. All scouts/scouters that may need a Part C for summer camp or high adventure are encouraged to attend.
January 28: 7:30 - 9pm
February 4: 7:30 - 9pm
Please bring the following to the event:
-COMPLETED health form
ALL sections completed and signed by an adult
Date of last tetanus booster listed on the form
-A copy of the front and back of your insurance card that can be attached to your forms
*Anyone with any recent new significant health events including seizures or heart conditions must be seen by their normal primary care physician.
*A parent must attend with scouts younger than 14 years old.
